Bethesda Row Arts Festival Application Form

Please fill out all information requested. All fields are required unless specified as optional.
Please upload your images in this form, with the title in the format, Lastname_1, Lastname_2 etc (with image #5 being your booth image) or, if necesary send them on disk via mail.
If you are applying for more than one category, you must submit two online applications or e-mail images and image information to info@bethesdarowarts.org.
To complete the application process, you must type your initals in the box below agreeing that you have read and understood the Artist Release form or follow the link to print our and sign copy, then mail or fax it back to our office with your jury fee. The application deadline is May 20th, 2011. Please make sure any mailed application forms are postmarked by May 20th(late-fee deadline May 31st, 2011).
